Want to have something to send clients between sessions to keep them accountable? Use this SMART goal form to have them write out their goal. They’ll share it with you automatically and can print it and tape it up on their fridge, bathroom mirror, or notebook. Clients will be prompted to sign at the bottom to acknowledge the pledge to themselves to accomplish the goal they have determined.

This form is a place where a client can write out their goal and go through the check-list of each acronym. Be sure you provide your client with some basic education on what a SMART goal is so they’ll have a handle on what it means. If they ever check “no” as they are going through each acronym, they’ll be prompted to revisit their goal. Once it’s saved, you’ll get it for review with the chance to comment back to them.

I also like to use these forms as reminders of how far they’ve come in working together. Even sending them a PDF of their first SMART goal after a month of working together can show them how far they’ve progressed. Great motivation!
