This handout provides practical ideas and information to help patients make lower sodium food choices by:

  • Identifying typical high sodium foods
  • Showing how to identify low sodium foods from the food label
  • Tips for making low sodium choices when eating out
  • Websites and apps to help patients identify high sodium foods at the grocery store and eating out
  • Suggestions on choosing this instead of that foods

Also included is a slide of a pie chart that can be laminated to help patients easily see the major sources of sodium in the diet.  This is especially helpful for those patients who feel that not adding salt is adequate for a low sodium diet.
