This is the waiver I have each client sign before the first session, outlining my policies.
This will come in an open word doc so you can personalize it with your logo or personal policies as necessary. When I send this to clients electronically I lock it so they can only access the fillable boxes to put their name and sign and date (to do this: go to the Developer tab and click the lock symbol that says Protect Form).
