This education handout is intended for use with the adult diabetes population.

As a nutrition or diabetes educator, naming snack ideas can start sounding like a broken record.

This worksheet allows the patient to:

  • Engage and consider their likes, dislikes, abilities, access to food and current nutrition knowledge to direct the conversation.
  • Explore their food preferences to develop balanced snacks to promote stable blood sugar.

Use in a group setting, support group, nutrition class or with one-on-one diabetes education and nutrition counseling. The worksheet is non-branded, allowing use in all clinic settings.
