This worksheet allows the patient to:
- Engage and consider their likes, dislikes, abilities, access to food and current nutrition knowledge to direct the conversation.
- Explore their food preferences to develop balanced snacks to promote stable blood sugar.
Use in a group setting, support group, nutrition class or with one-on-one diabetes education and nutrition counseling. The worksheet is non-branded, allowing use in all clinic settings.
I like how it is easy to understand and gives examples but also lets you pick and choose combinations as well!
